I hereby request that my child be accepted to attend fitness programs run by Every Body Fit LLC. I understand and am aware that my child will be participating in many physical activities and that the potential for accidents or illness does exist. In consideration of the acceptance of my child to such programs, I agree to the following:
RELEASE AND WAIVER
• On behalf of myself, my spouse, my child, and any executors and administrators, I hereby RELEASE, WAIVE, DISCHARGE, INDEMNIFY AND HOLD HARMLESS Every Body Fit LLC, its members, staff and volunteers for, from, and against any loss, damage, claim or cause of action of any nature whatsoever arising out of, or related to, any injury, accident or illness received or sustained or alleged to have been received or sustained by my child at Everybody Body Fit LLC fitness programs.
MEDICAL ATTENTION
• I grant permission for Every Body Fit LLC to provide or obtain medical attention for my child in the event of illness or injury and I understand accident insurance is not included in the fees. Should my child require medical treatment, prescriptions, or hospital care while enrolled in a program, I will bear the expense.
TERMS OF ENROLLMENT
1. Admission as a participant in Every Body Fit LLC fitness programs carries many privileges and responsibilities. We expect participants to engage fully in the program and to play well with other participants and staff. If a behavior or discipline problem affects our work with other participants, or their enjoyment of the fitness programs, Every Body Fit LLC reserves the right to dismiss those individuals responsible without a refund.
2. Permission is granted to use of any program photographs or video images in which my child appears for online or print promotional purposes of Every Body Fit LLC.
3. Each child must wear appropriate clothing and have tennis shoes or the child will not be allowed to participate. For all day camps, each child will need a marked water bottle, snack and lunch. (no heating or refrigeration available).
4. Refunds will be considered on an individual basis.
5. There is a $35.00 service fee for returned checks.
6. By typing your name in the signature box you agree you are signing this waiver form.
